Consent to personal data processing.

I consent to the collection, preserving and processing of my personal data contained in this form by the controller named above (or, if need be, the employees of the controller) for the purpose specified below. I give this consent for all the data contained in this form and for the whole period of 5 (five) years from the day of giving consent. At the same time I am aware of my rights according to Articles 12 and 21 of Act No. 101/2000 Coll., on the Protection of Personal Data. I have been acquainted with all parts of this form and all the data are exact and true and are provided voluntarily.

Advice according to Articles 12 and 21 of Act No. 101/2000 Coll.

The data subject can request information on the processing of his/her personal data from the controller. The controller is obliged to provide the data subject with this information without undue delay. In case the person who filled in this form discovers or presumes that the controller is carrying out processing of his/her personal data which is in contradiction with the protection of private and personal life of the data subject or in contradiction with the law, in particular if the personal data are inaccurate regarding the purpose of their processing he/she may ask the controller for explanation and ask the controller to remedy the arisen state of affairs; it can mean in particular blocking, correction, supplementing or liquidation of personal data. If the controller does not satisfy the data subject’s request, the data subject is entitled to appeal directly to the Office of Personal Data Protection.

The purpose for which personal data are to be processed.

In accordance with Article 5 of Act No. 101/2000 Coll., on the Protection of Personal Data all data contained in this form are collected and processed exclusively for the purpose of the provision of services and furthermore for the offering of products and services to the data subject. All the data contained in this form will be collected, processed and reserved also for the purpose of provision of commercial communications to the data subject via electronic services in accordance with Act No. 480/2004 Coll., on certain information society services, until the data subject directly and effectively sends the data controller information that he/she does not wish to be sent commercial communications anymore, or for the maximum period of 5 (five) years. The data subject further gives consent to attaching other personal data to the data provided. Summarized data from this form can be used by the controller for statistical purposes, more specifically for the needs of the controller itself.

Declaration of the controller.

The controller declares that the controller will collect personal data exclusively in an extent that is necessary for the fulfillment of the specified purpose and process them exclusively in accordance with the purpose for which they were collected. Employees of the controller, other natural persons who process personal data on the basis of an agreement concluded with the controller and other persons are obliged to maintain confidentiality of personal data. The obligation to maintain confidentiality shall survive termination of employment or the relevant work.
